Tundra Transmission out of gear with 4lo light flashing and beeping -Safety Issue

I have had an issue with my Tundra that has been documented since October 2011. Occasionally when I start the vehicle, the 4lo light is flashing accompanied by a beeping alert, the transmission releases into a neutral position and the shift selector becomes essentially useless (you can toggle from Park through each setting including drive and the transmission will not engage).
Two days ago (6/23/2015) I started my truck via remote start. The truck started and the issue above happened. The problem being that I was not in the vehicle and as the truck started, the transmission went into neutral and my truck began to roll. Although the parking lot was fairly flat, my truck gained enough momentum to crash into a parked vehicle. Lucky nobody was hurt, but the incident has got me thinking about how dangerous this issue is.
Back in October 2011, Toyota said they have never heard of the issue before and they could not replicate the issue on my truck as it happens randomly. It's done it maybe 100x since 2011, but certainly won't do it if you're trying to.

I would like to reach out to other forum members who have the same problem. If we band together, Toyota will have to recognize that it is a real problem and a real safety issue.

No solution. Just have to be careful with it. Toyota did some blackbox testing on my truck and then proceeded to "blacklist" me. Basically closing the case on the issue saying they wouldnt help.
This did cause an accident one day when my remote start fired up my truck while my key fob was in my pocked. No injuries, but over $6k of damage to a car my truck rolled into and the insurance co says it's an at fault accident. I wasnt even in the truck.

Hello everyone! Has anyone found a resolution or the underlying cause? This happened to me yesterday. I called 2 of my tech buddies from Toyota and they thought I was crazy! "It's impossible for the truck to just go into neutral"...ehhh not so much, fortunately I was in the truck when it happened and had the parking brake engaged.

Granted it has 320K miles but my "07 did this very thing last Friday. I've always had issues with the transfer case getting stuck in 4LO but this is a new thing for sure. Fortunately I don't have remote start. Local shop can't figure it out. If anyone has a definitive solution please put it out there.

Hello there, I have a 2007 Tundra and this is also happening to me, second time was today. Had to disconnect the battery terminal and reconnect to change mode of the vehicle so I could get home, I was 150km from home, I live in British Columbia, Canada. I searched the issue and found your forum, I can't believe this has happened to so many others.... I also was in 2 wheel drive. I stopped to get gas, and restarted my vehicle when 4LO started flashing and beeping then vehicle started to roll forward as I was on a slanted parking area, gear shifter was in Park. I will go see my dealer next week and share the info from all of you in regards to this situation. Thank you.

For all of you with this problem I've found what works for me every time is place the vehicle in neutral while running and push the 4wd nob in with 2wd selected hold down and wait till it beebs and the 4wlo light shuts off and u should be able to put it in gear and go
Don't know what the problem is but it happened for awhile when I first got the truck not so much any more

I have 2013 Tundra, driving on the freeway last night on 2WD, after about 5 miles 4LO came up flashing, doesn’t have any effect initially, i pulled over to slow lane planning on exiting freeway, suddenly transmission disengages, truck just rolling on the freeway with engine running but no drive. Shifted transmission to manual, transmission engages on manual, run on manual for a while, 4LO disappears, shifted back to drive “D” and was able to safely drive about 25 miles home. I have yet to scan/get inspection. What could be the problem? Thank you for any assistance.
