Surge/Stutter at constant RPM load

Hey guys, new to this forum. I have a 2011 DC 5.7 4x4. The only engine mods are the charcoal filter delete and a K&N dry air filter. A couple weeks ago, I started feeling a light stutter or surge under constant rpm loads. Cruising on flat roads at maybe 1500 rpm I can feel it lightly, but not all the time. If I give it more gas to go up a hill, holding say a steady 2,500 rpm, it’s more noticeable. Only about 3 times now in two weeks have I ever gotten an rpm change with the surge, and those 3 times have only been on hills where rpm’s were in the mid 2000 range. I thought it was the tranny at first, but have eliminated that. I’m guessing it could be the fuel pump, or sensors. Possibly a dirty MAF or TB. Anyone have this issue before? One other item, just in the last week, it runs rough at 1,000 rpm on startup after sitting overnight. Runs rough for 10-20 seconds. Then smoothes out. Only runs rough after sitting for hours, not on startup throughout the day.

Could be a lot of things. I take it the ECM has not thrown a code yet? When is the last time the plugs have been changed?.,, if recently did you replace them with the correct iridium plugs? I think you would get a misfire code by now it’s that noticeable,
The other thing that comes to mind but much more difficult to rule out is the torque converter.
@Brian20, I just replaced the plugs after the issue started with NGK Iridium IX. Problem didn’t change at all. No check engine lights. Transmission has been ruled out. I originally thought it was the issue, so I took it to a good transmission shop. They scanned the transmission and it does not have any codes. The owner also took it on a test drive with me and had his scanner hooked up. Torque converter was locking up correctly and everything. I did a transmission fluid change after that to see if that would take care of it. I dropped the pan and it looked good inside. Did a passive tranny flush using the method of taking the return line off the tranny cooler to cycle fluid through. Now all my tranny fluid is the new red color. Didn’t phase the issue.
Do u have access to a scan tool to do a live view of when the event is happening? If it is a misfire, you should be able to see which cylinder the ECM thinks is the issue by an increasing misfire count. Could be a coil pack failing as well.

I've mentioned my experience with a coil that liked to fail just enough to cause a very noticeable misfire, but not enough to throw a CEL in a couple of threads now. It liked to act up at about 1500 RPM, too. I could force a downshift and make it go away, but that probably shouldn't rule out a similar coil problem in the OP's case if other causes can't be ruled out.
If you don't have one already, get a cheap code reader. Look at mode 6 data to check the counts for misfires.
